Vball Indoor Academy –VOLLEYBALL WAIVER OF LIABILITY AND ASSUMPTION OF RISK.

WAIVER AND RELEASE OF LIABILITY – MINORS

In consideration for my child’s participation in any program, class, training, camp, clinic, event, or activity (collectively, “Activities”) offered, organized, or associated with Vball Indoor Academy, Vball Sport & Entertainment, Miami Beach Vball Booster Club, Miami Beach Senior High School, and/or Miami-Dade County Public Schools (collectively, the “Organizations”), whether paid or unpaid, I, as parent and/or legal guardian, agree as follows:

I voluntarily release, waive, discharge, and hold harmless the Organizations and their respective owners, directors, administrators, employees, coaches, instructors, volunteers, agents, and representatives (collectively, the “Released Parties”) from any and all claims, demands, damages, or causes of action arising out of or related to my child’s participation in the Activities, including those arising from the ordinary negligence of the Released Parties.

I understand that participation in volleyball and related Activities involves inherent risks, including but not limited to: falls, collisions, sprains, strains, fractures, head injuries, concussions, spinal injuries, illness, and, in rare cases, serious injury or death. I knowingly and voluntarily assume all such risks on behalf of my child, whether known or unknown.

I acknowledge that the Released Parties are not responsible for any injury, loss, or damage to person or property that may occur during participation in the Activities.

I authorize the staff of the Organizations to obtain or provide emergency medical care for my child if deemed necessary. I understand that I am solely responsible for any and all medical expenses, transportation costs, or related charges.

I affirm that my child is physically able to participate and that participation is voluntary with full knowledge of the risks involved.
